Derek McCulloch / President
Derek McCulloch brings 15 years of experience in the digital health marketplace to his role as the President of Primary Digital Health. Derek has served as the Subject Matter Expert, National Sales Manager and Director of Business Development for three separate digital health companies, where he led sales growth with the development of reseller networks, TPA sales, broker consultants, health plans and all areas of the payor ecosystem with a concentration on employee benefits. Derek shares Primary Digital Health’s vision of reducing the barriers to care by providing members with access to an efficient virtual-first approach that streamlines patient care while providing effective cost containment to the payor. His simple approach is built upon the understanding that better access leads to better outcomes.
Matt Davis / Partner
Matt’s passion lies in using strategy, innovation and technology to make healthcare more affordable and accessible for the very people who make organizations what they are. He brings this vision, along with years of industry experience, to Primary Digital Health. Matt has worked in in the healthcare industry for more than 20 years, serving in varying capacities including Medical Management, Fraud and Abuse, and Marketing and Sales. He has been the Director of Employee Benefits at RiskPoint since 2011.
Kelly Sanders, PT, DPT, ATC / Advisory Board
Board-Certified Clinical Specialist in Orthopedic Physical Therapy
Kelly serves as the president for Movement for Life Physical Therapy, which is an employee owned company operating 26 outpatient clinics in Arizona, California and North Carolina. She started her career at a certified athletic trainer and entered PT school at the University of Southern California while also working for their outreach program as a high school athletic trainer. Since graduating from physical therapy school, Kelly has completed an orthopedic residency program and is board certified in orthopedic physical therapy. She has worked in orthopedic physical therapy her entire career with a special interest in conservative musculoskeletal care as well as geriatrics.
Andrew Cherry / Advisory Board
Andrew Cherry serves as the Chief Operating Officer at Movement for Life, drawing upon nearly a decade of experience in the physical therapy industry. Prior to this, he held various positions in the public education sector, including roles such as Teacher, Principal, and Assistant Superintendent, as well as District Facilities and IT Director. Andrew’s current responsibilities at Movement for Life Physical Therapy encompass a wide range of areas, including HR, IT, Facilities, Marketing, Education, Business Development, and the Everflex health suite, which encompasses an array of software products and services.
Kallie Slette, PT, DPT, OCS / Advisory Board
Kallie Slette is the Director of Clinical Operations for Movement for Life. She attended the University of Idaho and completed the Sports Science program in 2010. In 2013, she earned her Doctorate of Physical Therapy from Upstate Medical University in Syracuse, New York. Kallie has a passion for high quality patient-centered care, progressing her team, the profession, and seeing conservative musculoskeletal care as a solution to many challenges of the current healthcare landscape.
Joe Godges / Advisory Board
Joe Godges is a Clinical Professor of Physical Therapy in the Division of Biokinesiology and Physical Therapy at the University of Southern California. Dr. Godges began his PT experience as an officer in the US Army in 1980. Joe is certified as a manual therapist, Rolfer, and PNF practitioner. His professional development also includes receiving his MA in counseling psychology. He was one of the original 29 individuals who first received board certification as a clinical specialist in orthopaedic physical therapy in 1989. He achieved recertification in 1999, 2009, and 2019. He has spent the last 33 years of his career training physical therapy residents and fellows in clinical residency and fellowship programs for Kaiser Permanente and USC. Dr. Godges has been the lead author or contributing author of over 20 peer-reviewed journal publications on several topics in orthopaedic and sports physical therapy.
Maury Hayashida, PT, DPT, OCS, SCS, CSCS / Advisory Board
Dr. Hayashida graduated from Western University of Health Sciences with a Masters and Doctoral degree in Physical Therapy after completing his undergraduate education in Kinesiology at Westmont College. Dr. Hayashida holds dual board certification by the American Board of Physical Therapy Specialties in both orthopedics (OCS) and sports (SCS) therapy. He is also a Certified Strength & Conditioning Specialist (CSCS) through the National Strength & Conditioning Association. He is currently pursuing his Ph.D. in Movement Science and Rehabilitation at Azusa Pacific University.
He has founded and operates 5 companies including a research non-profit and is a co-author of several human motion wearable technology patents. Dr. Hayashida is an adjunct faculty in the Kinesiology Department of Westmont College in Santa Barbara, CA. He is the former USA Rugby National Rehabilitation Coordinator and currently consults regionally and internationally with professional rugby teams.
